BREVITY COUNSELING
Brevity Counseling PLLC is a solo practice. Britney Vincent is licensed as a Licensed Professional Counselor in Texas and in Arizona.
This page is provided in compliance with Texas Health and Safety Code § 181.105, which requires covered entities to post instructions for requesting records, contacting the licensing board, and filing a complaint. It also satisfies 22 TAC § 884.31, which requires notice of how to file a complaint against a licensed counselor. That notice appears at the bottom of this page.
Arizona does not require a posting of this kind. Arizona clients receive the same information in their consent paperwork at the start of treatment.
Questions about anything on this page can be directed to Britney Vincent directly.
To request your records, email me at [email protected] or call 210-960-9076. I will send you a Release of Information form to complete, which will include a place to specify which records you want, your preferred format, and a copy of your driver's license for identity verification.
I'll respond within 15 days, sooner where your state's law requires it.
In rare cases, I'm allowed to say no, for example if giving you the records could put someone's safety at risk. If that ever happens, I'll tell you in writing and explain what you can do next.
There's no charge to get a copy of your own records.
